Patron Instructions

Fill out the form below to apply for your free library card!

• You must be 18 or over to apply for a digital full-access card.


• You cannot apply for a digital card here if you already have a library card.


• If you want a physical library card, you must visit the library in-person.


• When you apply here, the system will confirm your identity and that you reside in Adair, Cherokee, Delaware, McIntosh, Muskogee, or Sequoyah counties in Oklahoma.


• If you are under age 18, a responsible party will need to visit a branch and show ID and proof of address to obtain a card.


• If we are unable to verify your residence/identity through this form, you will receive an email with further instructions on how to obtain a card in person at your local library.


• Once your digital card is approved, you will receive a confirmation email with instructions on using it, including accessing it through the EOLS free app or adding it to your Google or Apple Wallet.

• I agree to follow the EOLS Library Users Policy and Internet Policy and accept responsibility for paying replacement costs for any lost items checked out on my or my minor child's card. (EOLS libraries do not charge late fees.)
